We are currently seeking Certified Nurse Aides to support our TRISUN - Gracy Woods I team in Austin, TX!!

SUMMARY

Ensures that resident's personal care needs are being met in accordance with Residents' wishes and Care Plan Objectives.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Answers call lights, bells, or intercom system to determine residents' needs and ensure call lights are within reach of resident.

Ensures that residents who are unable to call for help have an alternative help notification system or are checked frequently.

Participates in Care Plan Objectives.

Gives direct resident care such as bathing, dressing, and grooming of residents.

Documents care provided in CARE Tracker system and/or flow sheets.

Explains procedures to residents before administering.

Assists residents as necessary in their activities of daily living.

Assists residents with bowel and bladder functions.

Assists residents in preparation for and transporting to appointments, activities and social programs.

Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs as noted on resident's DOT system.

Keeps residents dry by changing gowns, clothing, linens, etc. when they become wet or soiled.

Dresses resident appropriate to the current weather.

Notifies Social Services if resident has personal or psycho-social needs.

Reports to DON/Administrator alleged incidents/accidents and alleged abuse incidences and complaints per State and Federal regulations and company policies.

Completes grievance form and turns in to Social Services any complaint or grievance.

Changes bed linens and keeps linens tight to avoid wrinkles from forming under residents.

Handles linens according to infection control guidelines.

Serves and collects food trays and feeds residents requiring help.

Delivers and documents all a.m., p.m., and bedtime snacks and supplements.

Offers fluids to residents frequently to ensure that residents are hydrated where designated by care plan.

Reviews and implements daily nursing needs as outlined by resident Care Plan or nurse managers.

Transports residents or assists residents to walk.

Turns and repositions bedfast residents, alone or with assistance, to prevent bedsores.

Takes and records temperatures, blood pressure, pulse rates, respiration rates, height, weight, and food and fluid intake and output, as directed.

Monitors resident's activities and reports any irregularities of resident performance to nurse managers.

Participates in and receives nursing report upon reporting and departing to/from work.

Reports all changes in residents' conditions to supervisor as soon as practical.

May function as a mentor for other CNAs, orienting them to their position.

Completes appropriate charge and inventory slips and monitors supply inventory daily to see what is needed; cleans instruments and equipment and returns to proper area after use.

Follows infection control practices and maintains cleanliness of residents units, central bathing area, treatment and supply rooms, workrooms, and linen closets.

Provides post-mortem care as instructed.

Attends and participates in scheduled in-service programs and staff meetings.

Maintains confidentiality of necessary information.

Understands and adheres to the guidelines of "Residents Rights".

Reports observed or alleged allegations of neglect or abuse, per state and federal regulations and Company policies.

Completes grievance form and turns into social services for any complaint or grievance.

EDUCATION AND EXPERIENCE, SKILLS, CERTIFICATES, LICENSES, AND/OR REGISTRATIONS:

Education: High School Diploma or GED.

Experience and Skills: Prior experience as LTC nursing assistant preferred but not required.

Certificates/Licenses/and/or Registrations, etc..: Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program.

Good standing in the state registry is required.
